Assessment and Support Planning Officer 

Merton Team for People with Learning Disabilities

Grade/Salary: ME10  £30,930 - £32,637 per annum

Fixed Term/Secondment for 1 year

An exciting opportunity has emerged in the Merton Learning Disabilities team following the secondment of a member of our team. 

The Merton Team for People with Learning Disabilities is a supportive multi-disciplinary team who are committed to using a person-centred approach to promote a good quality of life for individuals with learning disabilities. Our team includes social workers, specialist community nurses, Assessment and Support Planning Officers, psychologists, speech and language therapists, occupational therapists and physiotherapists under a single management system. Our team has a long history of working well together, and it is this experience that allows us to deliver an exceptional service to both our customers and their carers. We have been praised by the CQC who were very positive and rated our service as “good” on all their criteria.

There has never been a more exciting time to join Merton’s Adult Social Care Services. We have successfully completed a restructure of our services, ensuring we can work in a more customer focussed, flexible way and focus our interventions towards prevention.

Through investing in our teams and preventative services, ensuring effective and reflective supervision and implementing our new social care case work system, we believe we are creating a working environment where you can help us deliver the improved life choices and life chances for our most vulnerable residents.

You will be enthusiastic, creative and flexible and have excellent communication skills. Some experience of working with adults with learning disabilities is preferable. However we would welcome candidates who would be interested in developing their skills in this area and are willing to give the appropriate training and support to enable you to do this.
